You will also need driver’s license information for the operators and the license plate numbers of the vehicles involved. If the vehicle’s owner is not driving it at the time of the accident, you will also need the name and address of the registered owners. Other requirements include the name and address of the insurance company for any needed policyholder; a write-up about the accident that includes date, time, and location; and a narrative explaining how the accident occurred. Take photographs at the scene even though the report may not require it.
Keep in mind, Texas Transportation Code §550.062 dictates reporting for law enforcement officers. If you need to file a report on your own, you would be required to use Crash Report Form CR-2 also known as “the Blue Form.”
